Institutional Review Upload Templates  - The Institutional Review Committee has created these templates for Sponsoring Institutions to use as the basis for the Institutional Review Upload Documents. The templates include the elements indicated in the Institutional Requirements. Use of these templates is optional, and Sponsoring Institutions may use any other compliant format that meets their local needs. The documents are provided in an editable format so Sponsoring Institutions may customize them as needed.
